Payment Options 

University Surgical Associates (USA) requires payment of all insurance copays at the time of service, including those covered by a Preferred Provider Organization (PPO) or Health Maintenance Organization (HMO).

If you need assistance in determining if you're covered by a PPO or HMO, please call your insurance company (the number is usually on the back of your insurance card). We also accept Medicare assignment of benefits. Your Medicare carrier determines the approved charge under “Part B” and will pay 80 percent. The patient is responsible for the remaining 20 percent, but that may be covered if you have a Medicare supplement plan.

For your convenience, we accept Visa, MasterCard, Discover, American Express, cash, personal checks or cashier's checks. A $20 fee will be applied to all returned checks.

USA also offers an online payment feature for all outstanding balances through a secure, encrypted portal. Visit our Payment Portal to pay on your account from the comfort and convenience of your own home.
