Disability Forms

University Surgical Associates (USA) is happy to assist you with the completion of any appropriate disability forms. In order to defray our administrative costs, we do require a $10 payment for this service. 

Patients may submit forms for completion during normal business hours (Monday through Friday, 8:30 a.m to 5 p.m.). Completion of disability forms may take up to 14 days. Once forms are complete, a USA representative will contact you and schedule a time that the forms may be picked up or, if instructed, will mail completed forms to your home address.
