Schedule an Appointment 

When scheduling your appointment at University Surgical Associates (USA), please be prepared to provide the scheduling operator with your insurance information and other personal data, including your address, the name and phone number of your primary care and/or referring physician and contact information. Please inform the scheduler of the nature of your visit to allow sufficient time for your appointment. 

Twenty-four hours in advance of your appointment, a USA scheduling operator will call to confirm. Please make certain to provide a working home or cell number when scheduling your appointment.

If you or your child is a first-time patient at USA, you can complete pre-registration forms online. This will minimize your wait time at your first appointment. If you choose not to pre-register, please arrive between 15 and 20 minutes prior to your scheduled appointment time.

In order to provide the best possible care for all our patients, we ask that you notify us 24 hours in advance to cancel or reschedule your appointment by calling our main switchboard at (423) 267-0466.

USA surgeons and staff make every attempt to see patients promptly, however, emergencies can arise that may delay your appointment. If your surgeon is involved in an emergent situation, we may need to reschedule your appointment. We will contact you as soon as possible and arrange for another appointment time.
