Films and X-rays 

Your films and X-rays are kept in strict confidence as part of a permanent file University Surgical Associates (USA) maintains on your behalf. USA will only release copies of your records if we have a completed Medical Records Request Form. USA does not charge its patients for the transfer of medical records.

If you are not in possession of your medical records, please contact your physician's office and request your records be sent to USA. We may need your written permission in order for the records to be transferred. If you are not able to have your medical records sent in time for your first appointment or are unable to bring them with you, we may need to reschedule your appointment.
