If your insurance provider is out of network with USA, we will provide your care; however, you will be responsible for any fees your insurance provider does not cover. Since many of our services are unique and are not provided elsewhere in the community, your insurance company may grant a waiver to allow us to care for you, even though we may not be listed in your plan, . Please contact our Billing Office at (423) 265-6578 if we can provide assistance in these matters.

To keep our billing expenses and fees as low as possible, we ask that all co-pay and deductible charges be paid at the time service is rendered, including those covered by a Preferred Provider Organization (PPO) or Health Maintenance Organization
(HMO). 
For your convenience, USA accepts Visa, Master Card, American Express and Discover, cash or checks (a $20 fee is charged for all returned checks). If you need assistance in determining if you're covered by a PPO or HMO, please call your insurance company. 

USA also accepts Medicare assignment of benefits. Your Medicare carrier determines the approved charge under “Part B” and will pay 80 percent. The patient is responsible for the remaining 20 percent, however, if you have a Medicare supplement plan it may cover this charge. Medicare Advantage plans typically require a co-pay or deductible in lieu of an 80/20 split.
