Referral Instructions 

Because of University Surgical Associates' scope of practice, many insurance companies require a referral from a primary care or other physican/clinician for patients to be seen by our surgical staff.

Please verify with your insurance company if they require a referral. If so, please be sure to provide the physician's name and office phone number at the time you schedule your first appointment.

It's possible your appointment may need to be rescheduled if we do not have the proper referring information.
